Criteria For Services
Adults (ages 18+) who demonstrate a risk for out of home placement, psychiatric hospitalization or isolation from social supports as a result of serious mental illness (i.e., Major Depressive Disorder, Bipolar Disorder, Schizophrenia or other psychotic disorder).
Individuals who have significant deficits in independent living skills (i.e., money management, hygiene, food shopping and preparation, household management, organizational skills, etc.) such that they are unable to maintain stable housing or are at risk for homelessness as a result of their mental illness.
Individuals who are currently or have been prescribed psychiatric medications within the past 12 months and who are willing to comply with psychiatric services.
Individuals who have a history of intensive psychiatric services (i.e., psychiatric hospitalization, PACT or ICT interventions, crisis stabilization or temporary detaining order).
